There are several hospice programs providing care in this area. Check to see which one you prefer to use. Your doctor and hospital discharge planner must honor your choice if you are a Medicare beneficiary. This is true regardless of the HMO or managed care program you use. You also have the right to transfer from one hospice to another in any benefit period.
Hearts for Hospice serves residents in Utah, Arizona and Idaho. We are affiliated with regional and national Hospice organizations, but all of our services are devoted to local residents.
The doctor or hospital discharge planner can contact Hearts for Hospice directly, or a patient, family or community member may also call.
Hearts for Hospice must have the consent of the patients primary physician before beginning services.
Questions to ask before choosing a hospice include:
- What is the ratio of patients to staff?
- Do you have and utilize volunteers?
- What do they do?
- What type of bereavement services do you offer?
- Does your hospice have music therapy?
- Will your hospice treat my love one as if it was their own family member
- Can your hospice admit your love one within an hour?
- Does your hospice have their own transportation?
